MCHC (Hemoglobin Concentration)

Table of contents

MCHC tells you how tightly hemoglobin is packed inside your red blood cells, expressed as a concentration rather than a total count. It is one of the red cell indices reported on your complete blood count and helps your clinician sort out what type of anemia you might have.

What MCHC measures

MCHC is the average concentration of hemoglobin within a given volume of red blood cells, calculated from your hemoglobin and hematocrit values. Do not confuse it with MCH, which measures the average amount of hemoglobin per cell rather than its concentration. Because MCHC accounts for cell size, it can flag whether your red cells are hemoglobin poor or unusually dense.

What high or low levels mean

A low MCHC means your red cells are hypochromic, or paler than usual because they hold less hemoglobin. This most often points to iron deficiency, and sometimes to thalassemia or chronic disease. A high MCHC is uncommon and can suggest hereditary spherocytosis, a condition where red cells are small and dense, or it may be a lab artifact from cold agglutinins, clumping, or a sample handling issue. If your MCHC sits outside the reference range, your clinician will usually look at it alongside your MCV, MCH, and iron studies rather than in isolation. Follow up when a low value comes with fatigue, or when a high value repeats on a fresh sample.

MCHC FAQ

Is a low MCHC always iron deficiency?

No. Low MCHC most commonly reflects iron deficiency, but thalassemia, anemia of chronic disease, and lead exposure can also lower it. Your clinician confirms the cause with iron studies and other red cell indices before deciding on treatment.

Can dehydration change my MCHC?

Dehydration mainly concentrates your overall blood and shifts hematocrit, but MCHC stays relatively stable because it already accounts for cell volume. A surprising high reading is more often a lab artifact than a true physiologic change, so repeat testing helps.

What does a falsely high MCHC mean?

A falsely elevated MCHC usually comes from technical issues such as cold agglutinins, lipemia, or red cell clumping in the sample. The lab often flags these and reruns the specimen, since a true MCHC above the normal range is rare.

Read alongside your MCV, MCH, and red blood cell count results for a fuller picture of red cell health.
